Name, address, phone, birthdate and social security numbers of ALL family members. You may not want to add them to medical, but may want to add them to other benefits, so it is better to have their information loaded.

Beneficiary Info - the same information as above is needed. Generally, spouse should be the main beneficiary on life insurance polices. It is not suggested to make children under 18 a beneficiary, so please have the information of an alternate person with you.
